Sometimes, situations can arise when some short term but intensive support is needed, with a view to shortly returning home permanently, an either avoiding the need to go into hospital at all, or moving out of hospital sooner than a return to home is otherwise possible.
This provision of care can be more intensive, and often involves support from a range of visiting NHS professionals, delivering services together with the home’s team in an environment that supports the resident to be as independent as soon as possible.
Care planning is undertaken and agreed with the resident and / or family as appropriate by the nursing team and specialist nurses, and the home team ensure that planned care is followed through.
Residents who stay whilst receiving intermediate care are encouraged to participate as much as they wish in what is happening in the home so that they feel as confident as possible to resume their independent life when they return back to their own home.
Close liaison and communication is maintained with families as appropriate during this time to ensure that everyone involved can support the resident fully during this time of transition.
